| Corporations providing large scale teleradiology services are aware of the changing legislative environment upon which they must build their companies. While previously introduced bills complicating oversea transmission of personal medical data have recently died in committee, there does remain a possibility of revival, particularly through a Democratic renewal in Washington. Contending issues collide when these corporations desire to offer an international option to professional employees, yet uncertainties of the ramifications of this option remain. The increasing request for “final reads” by clients add another layer of complexity as Medicare rules clearly limit reimbursement to studies interpreted within the USA. Overseas transmission of data also is under a cloud, as on the event horizon of our profession stands the specter of “ghost reading” and inadequately trained Indian radiologist undercutting our markets. A single competent USA trained national could leverage himself thereby into multiple hundreds of daily reads. Because of these concerns many Corporations discourage international placements and use the American flag in marketing as a feature for differentiation from competitors. It is evident, although, that the most successful corporations do offer an international option freely. These restrictions thereby place large numbers of employed physicians on a vocational path with working hours that no individual could desire long term. Multiple and expected personal life changes of each radiologist does produce a renewed evaluation of this “night shift” position. With this second look, an important corporate investment is at risk.
