| Overcoming the weight of 21st century call demands has required creativity on the part of every USA radiology practice. The combination of tremendous day volumes, exhausted “on call” members, and the specialty requirements of each practice combine often to create an impossible condition. Optimal recruiting strength requires an attractive package and in today’s tight marketplace the applicants call expectations is an issue that often determines success. A solution may be found by having critically important members on call in rotation, with no day requirements during this time. This solution creates a service deficit and decreases overall professional productivity. One may hire individual “night hawk” radiologist to relieve this stress, yet these individuals are difficult to find, require lengthy credentialing in all necessary facilities and have a proclivity to seek more suitable day time positions. The perfect solution seems to be the addition of a teleradiology corporate service company, if one can be assured of the quality and timeliness of their work. This corporate presence becomes an extension of the group’s practice and does influence the group’s overall reputation. This solution offers benefits, but for large group two issues arise; the expense and the possibility of loss of control as you credential and network many capable licensed professionals within your home practice. The current lack of clarity of the future of radiology practice patterns gives one pause as one considers these corporate services with expertise beyond you own and economies of scale that could influence third party payers. Contracts could become fragmented at the commercial contract level with premium cases diverted to “less expensive” mass providers of professional services. It often becomes unattainable to find a perfect solution for all of these competing needs and compromises are made while one hopes for the best.
